MINOR release/Parental consent form needs to be signed for all practice and races at Mesquite Mx Park. Parental Consent 1 Parental Consent 2
No one under the age of 18 is allowed to practice or race at
Mesquite Mx Park without a parent or legal guardian signing and
notarizing the parental consent form. Parents can print both sides,
sign and notarize the waiver before child is allowed to practice or
race without the parent present. The parent or legal guardian must
also attach an enlarged copy of their drivers license or photo
I.D. Anyone under 18 that comes to practice or race at Mesquite Mx
Park without parent or legal guardian must have our parental consent
form signed and notarized for every race and practice.
